• Application for Senior Membership in Civil Air Patrol

  This application form (CAPF-12) can be filled out with MS/Word (on your computer).     Additionally, you must obtain a special fingerprint card from SQ. HQ, be fingerprinted, and pay a $68 application fee (check payable to Civil Air Patrol).

  Three things:     1) The Membership Application, 2) application fee and 3) finger print card must be returned to Sq. HQ.     The application must be signed, front and back, by you and the Sq. Commander or designee before mailing to CAP NHQ Membership Office.     Once approved, Squadron membership dues and minimal insurance premimums are payable (please inquire for more information).
